If you choose to utilize another compounding pharmacy, please see below for the needed information in order to process this request. Please also note that very strict legal requirements need to be met in order for our veterinarians to authorize a compounded medication. Cost of that medication is unfortunately NOT a reason to authorize a compounded medication when an FDA approved medication is available for your pet. Thank you for your understanding.
Submitting an OUTSIDE PHARMACY Prescription Request
Please note that if you choose to utilize a different outside pharmacy (such as Chewy, 1-800-PetMeds, etc.), we need you to complete the medication/prescription diet request/refill form and submit that to our office via text or email. We CAN NOT complete requests submitted by the pharmacy directly. We can only submit a prescription based on your request to our office for the prescription to be filled by your chosen outside pharmacy. In addition, any transfer or changes to that prescription need to be addressed by you to the pharmacy directly. Our staff is not able to do that for you. Finally, due to the large amount of time our staff takes to ensure that your prescription requests are correct and safe for your pet, we are happy to submit a prescription to an outside pharmacy once. Any need to re-write or adjust a prescription will incur an additional cost, so please be as accurate as possible on your requests to avoid any additional charges. Thank you for your understanding as our entire team works hard to make sure your pet has the necessary medications for a reasonable cost.
Information needed for Prescription Request to outside pharmacy:
1. Pet Name
2. Client Name, Address, Phone, and e-mail
3. Medication Requested (please be as specific as possible such as chew, tablet, capsule, any flavoring, etc)
4. Strength of the Medication (ex. 10mg tablet)
5. Quantity of medication requested and if any refills would be needed (ex. 30 tablets with 1 refill).
6. Amount and frequency that your pet is currently getting the medication (ex. 1 tablet by mouth every 12 hours).
7. Name of Outside Pharmacy along with Address, Phone, Fax, and E-mail of Pharmacy
8. Please list all other medications your pet is currently taking for our staff to ensure that there are no interactions with the requested medication.
Please allow 3-4 business days to process this request. Please contact our office with any additional questions. Please note that our office will send this prescription once and any requested changes either need to be addressed with the pharmacy directly or if re-submission of prescription required, it will incur additional charges. Thank you for assisting us with your pet's medications.
